Throughout the patient discharge process, the comprehensive, reliable and standardised medicines data within the Multilex Drug Data File (Multilex DDF) can be used to populate discharge summaries, verify TTO prescriptions, produce pharmacy dispensing lists and support labelling for new and existing medications. It can also be used to capture clinical information at the point of admission, saving time at discharge.

With a hospital’s entire clinical team – doctors, nurses, pharmacists, discharge team - accessing and using standardised data from a single, trusted, electronic source, medicines can be recorded in a standard and consistent format, reducing prescription errors and improving patient safety.

Electronic discharge - data set
Multilex DDF’s drug data exceeds the minimum data set requirements for medicines reconciliation and electronic discharge in secondary care and includes:

- an industry approved pick list of all UK medications and appliances from which the items relevant to local formulary can be selected
- links to the NHS dictionary of medicines and devices (dm+d) to support messaging and translation
- a list of age-appropriate, indication-specific doses
- information on medication, route, formulation and frequency
- patient counselling information

The comprehensive drug data allows users to build a customised prescription manually using drug name, routes and, where applicable, base formulations and strength.  The licensed status of the medicine, its route(s) and indication(s) are all available to review. To complement this First DataBank's Dose Module offers users a number of suitable, pre-built dose suggestions. These reduce reliance upon memory and the risks associated with manual entry of numerical dosage values and units. For users wishing to create their own dose the Dose Range Check will alert if the bespoke dose is outside a safe range for a given medicine.

The Clinical Documentation Solution Accelerator for the Microsoft Office system is a Connected Health Platform resource that enables Trusts to define and create clinically encoded documents. Documents can be created based upon ready-made templates (for example, for a Discharge Summary or a Referral) retrieved using the Microsoft Office System.
